Wellframe makes managing your health easier by connecting you to care right on your iPhone. Wellframe is available exclusively through your healthcare provider, and can be used for any condition or disease. When you enter your personal Wellframe code, you’ll receive your clinician-approved care plan as a daily multimedia checklist. The Wellframe app gives you access to exclusive content to help you learn about your condition, review your progress towards your health goals, and communicate with your Care Team. The information you enter on Wellframe will be shared securely, in real-time with your Care Team to review and provide supportive feedback to help you reach your health goals and engage in healthy living.
